: This study was conducted in compliance with the Declaration of Helsinki.<sup>25</sup> The participating GPs consented to participate by clicking on the survey link in the invitation email, and responded anonymously. In Brazil, the Netherlands, and Switzerland, the ethics committees issued a waiver (Albert Einstein Ethics Committee under the register 62,920,716.2.000.0071; Committee of Medical Ethics of the Leiden University Medical Centre, and the Kantonale Ethikkommission Bern, respectively) and in New Zealand, the University of Auckland Human Participants Ethics Committee approved the amendment to the original ethics application of the previously conducted study by this group (with similar design and content)<sup>11</sup> for this study. Ethical approval was not required in the other countries.
