: All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. The study and data linkages have been approved by the Regional Committee for Medical and Health Research Ethics South East Norway (REC South East, reference number 15538), Statistics Norway, the Norwegian Directorate of Health, and the Norwegian Institute of Public Health. The data were handled in accordance with the General Data Protection Regulation, and a Data Protection Impact Assessment has been conducted in collaboration with the Data Protection Officer at the Norwegian Institute of Public Health. The study was based on national health registries to which reporting is mandatory and obtained exemption from confidentiality from REC South East, in accordance with the Norwegian Act on Medical and Health Research (the Health Research Act).
