: The study was presented to the medical ethics committee METC Z who waived the need for ethical approval under Dutch law. The study has been performed in accordance with the ethical standards as stated in Dutch law and the regulations of the Dutch Institute for Clinical Auditing. The study protocol was approved by the scientific committee of DATO. No informed consent was needed as DATO is an opt-out registry. For the EPF research, a waiver for additional informed consent was obtained by the local medical ethics committee.
