: The study was performed as an individual diagnostic pathway per patient in consensus with the patient and was approved by the local Ethics Committee of the University Medical Center Goettingen (approval June 7, 2015). All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. For this type of study formal consent is not required.

: The institutional review board of our institute approved this retrospective study, and the requirement to obtain informed consent was waived. All procedures followed were in accordance with the ethical standards of the responsible committee on human experimentation (institutional and national) and with the Helsinki Declaration of 1975, as revised in 2000. Informed consent was obtained from all patients for being included in the study.
