: We submitted this process evaluation to the Medical Ethics Review Committee of the Academic Medical Centre (AMC), Amsterdam, for its approval under registration number W17_312#18.365. The committee confirmed that the Medical Research Involving Human Subjects Act did not apply to the study and that the committee’s formal approval was therefore not required.

: All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards.
: Informed consent was obtained from all participants in the GIRONA study. All participants were informed at the beginning of the study that information would be collected for scientific reporting purposes. Only those patients who indicated on the GIRONA informed consent form that they were approachable for further research were invited to complete the additional process evaluation questionnaire.
