: The study was approved by the local ethics committee of a Tertiary Portuguese Care Center (Centro Hospitalar Universitário São João), Porto, Portugal, on under the reference CE-189-2011 and conducted in accordance with the 1964 Helsinki Declaration and its subsequent amendments. The initial approval was granted in 2011, and an amendment allowing the performance of skin biopsies was approved in 2015.
