: Research permission for data collection and analysis of this register-based retrospective study was granted by the Institutional Review Board of Turku University Hospital, Turku, Finland. The study design was approved by the Office of the Data Protection Ombudsman. The study was conducted in accordance with the Declaration of Helsinki during the period 2014–2016. All personal data was assigned a computer-generated code, and the identity of the patients was unknown throughout the study.
