: Obtaining informed consent was considered unfeasible for this clinical audit, both because many of the patients were already dead at the time of data collection and due to difficulties in contacting the rest. However, the institutional ethics committee approved the study protocol. Personal (identifying) information was stored in one database and clinical data for analysis were entered in another; confidentiality was guaranteed by Dr. Manchon-Walsh.
