: This study was approved by the Majorca Primary Care Research Committee (PI11/12) and by the Balearic Islands Ethical Committee (IB11-3367). All participants received information about the study during the initial interview and provided signed informed consent before participation.
: The GPs contacted the individuals in both groups by telephone to request participation in the study. An interview was arranged in the health centre or at home, and each participant signed an informed consent document.
