Our Consent of Care states the following: “I recognize that research will not be conducted on me unless I voluntarily give my informed consent, or as otherwise permitted by laws governing research. I consent, however, for my Health Information to be used consistent with federal law in reviews preparatory to research, in research studies if I should be deceased, or in research where the Institutional Review Board and Privacy Board have approved the use of my Health Information for research.”
: For this type of study, consent for publication is not required. Our institutional offices for Scientific Publishing and Corporate Compliance were contacted and informed. As per our institution policies (policy no. 102.3) that references the published “John Hopkins Medicine Organization Policy on Single Case Reports and Case Series,” authors who remove HIPAA identifiers (including unique patient characteristics) from the data prior to submission and publication of the article do not need to obtain a signed privacy authorization. This case report does not contain any HIPAA identifiable data. The patient is deceased. An additional specific case report consent form is not recommended or needed in this situation.
