: The survey was not reviewed by an ethics committee and due to the nature of the survey and information collected is exempt from this requirement based on the Human Subject Regulations Decision Charts: 2018 Requirements: 45 CFR 46.104(d)(2) for Educational Tests, Surveys, Interviews, or Observation of Public Behavior. The trial was conducted in accordance with the principles of the Declaration of Helsinki of 1964 and its later amendments and Good Clinical Practice guidelines of the International Council for Harmonisation. Before enrollment of patients, the study protocol and informed consent form were reviewed and approved by an appropriate Institutional Review Board or Independent Ethics Committee.
