: The original trial received ethics approval from the Central Health and Disability Ethics Committee (11/09/054) and was registered with the Australia and New Zealand Clinical Trials Registry (ANZCTR: 12611000862943). Written informed consent was obtained from all parents/caregivers on behalf of trial participants, and verbal assent from participants. Ethics approval to use de-identified data from the interim service was not required, as clinical audit studies are exempt from full Health and Disability Ethics Committee review under section 3 of their Standard Operating Procedures v.3.0 (Ministry of Health 2019).
