: This study was approved by the Ethical Committee of Yazd University of Medical Sciences. We described the purpose of our survey to the respondents and they filled the questionnaire voluntarily. The questionnaires have been anonymous and only used for research purposes. Ethical Committee did not request that a written informed consent form was needed to apply the questionnaire.
