: This study was conducted at four institutes and was approved by the Ethics Committees of three institutions (Jichi Medical University, The Institute of Medical Science of the University of Tokyo, and Saitama Medical University International Medical Hospital). The Ethics Committee of the fourth institution (National Cancer Center Hospital) determined that ethics approval could be waived because the study participants were nurses, rather than patients. This study was performed in accordance with the ethical standards as laid down in the 1964 Declaration of Helsinki, its later amendments and Ethical Guidelines for Medical and Health Research Involving Human Subjects in Japan.
