: All participants provided informed consent indicating their data will be used for medical research purposes and the study results may be published. The studies were performed in accordance with the Helsinki Declaration of 1964 and its later amendments. Ethical approval and oversight for the qualitative interviews was obtained from Western Copernicus Group Independent Review Board (WCGIRB; reference ADE1-17–162). Ethical approval for the psychometric validation activities was obtained as part of the phase 2b trial (NCT03683719) from the independent institutional review board (IRB) and ethics committees listed in Table .
