: The survey was performed in compliance with the European Pharmaceutical Market Research Association (EphMRA) [] and in full accordance with the US Health Insurance Portability and Accountability Act (HIPAA) 1996 [] and the Helsinki Declaration of 1964 and its later amendments. Ethical approval was granted by the Western Copernicus Group Institutional Review Board (WCG-IRB, approval number AG8836). Using a checkbox, patients provided informed consent to take part in the survey. Data were collected in such a way that patients and physicians could not be identified directly.
