: The lead author repeatedly attempted to contact the relatives of the patient for review of the manuscript, but was unsuccessful in making contact. Consent for publication was therefore not obtained, but patient information and descriptions were sufficiently anonymized in accordance with ICMJE guidelines, with no use of external or identifiable photographs. Moreover , the Kitasato University Ethics Committee determined that the submission of this case to an English-language journal and presentation at academic conferences does not conflict with the “Ethical Guidelines for Life Science and Medical Research Involving Human Subjects , ” even if the individual is deceased and consent cannot be obtained (Approval No. B24-114).
