: Blood samples from patients and controls were collected after obtaining informed and written consent from each participant. The study was approved by the local ethic committees; in India by the CMC, IRB & EC (Institutional Review Board & Ethic Committee, Christian Medical College, Vellore) and the Health Ministry Screening Committee, ICMR (Indian Council of Medical Research), in Norway by the Regional Committee for Medical and Health Research Ethics. The study was conducted according to the ethical guidelines from the Helsinki declaration.
