All patients gave their informed consent for participating in the study, pseudonymized data evaluation, and publication. The structured interviews to collect the patients’ medical history were administered as part of our standard procedures in patient’s care. The study was approved by the ethical committee of the University Hospital Heidelberg (approval number 487/2011).
