: All study procedures were conducted in accordance with the ethical standard of the WCG Institutional Review Board (IRB) and the Helsinki Declaration of the World Medical Association. The qualitative study was reviewed by SCG IRB and determined exempt (#1365464). The quantitative study was reviewed and approved by WCG IRB (#1377343). Qualitative participants were provided with a study information sheet and gave verbal informed consent, including consent to audio recording and publication of de-identified findings, prior to interview initiation. Quantitative participants provided electronic informed consent prior to survey participation. Data from both phases were de-identified to protect participant privacy and confidentiality.
