All procedures followed were in accordance with the ethical standards of the responsible committee on human experimentation (institutional and national) and with the Helsinki Declaration of 1975, as revised in 2008. <i>Consent to participate:</i> Consent was obtained from the parents of children/adolescents aged under 16 and assent was obtained from the young person. Written informed consent was obtained from adolescents aged 16 and over and their parents/guardians were informed of their participation. <i>Consent for publication:</i> On the consent/assent form, patients/guardians were asked whether they consented to anonymized data being published in an academic journal.
