: All authors contributed to the study conception and design as well as material preparation. CEJ was the project manager and contributed to the Danish translation of the training material, trained the interviewers, managed the data collection, was responsible for the bi-monthly quality control reports, and contributed to the data analyses. SSS and CG tested the training material and trained interviewers. SSS also contributed to data collection. MBJ performed the data analyses and discussed these with the EQ-VT support team.
