: This study was exempt from review by the Medical Ethics Review Committee at the University Medical Center of Groningen. It issues waivers stating that the pertinent Dutch Legislation (the Medical Research Involving Human Subjects Act) does not apply to survey research where participants fill out a single questionnaire, which does not pose additional risks. A third-party vendor, Dynata, collected data of this study from Dynata’s panel of patients in the USA. Dynata complies with the ethical guidelines and standards set forth in the Market Research Society Code of Conduct.
