: The study protocol was approved by the Institutional Ethics Committee of the GMERS Medical College, Gandhinagar (letter no. GMERS/MCG/IEC/19/2017, dated 1 March 2017). The entire study was conducted following Indian Good Clinical Practice guidelines (Schedule Y).
: The purpose and nature of this study was explained clearly to participants in their vernacular language, and written informed consent was obtained from all participants prior to enrolling them into the study.
