: LH leads the BRIDGES program of research. AH led the writing of this manuscript. LH designed the study protocol in collaboration with AH (qualitative methods) and KH (health economist, discrete choice experiment expertise) and feedback from LJ, GW, KC, ST, AS, SC, GV, RA and CS. LH, LJ and AH conducted focus groups and interviews with a paid qualitative project officer. LH, AH, LJ, SC and BW formed the core research group who met weekly to discuss progress with additional meetings with the broader authorship team when needed for iterative deliberations about aspects of the DCE blocks and/or survey instrument.
