Participants had the right to withdraw from the survey at any time. Informed consent was obtained from all study participants before phases 3 and 4 of the study. The study protocol and informed consent form were submitted to an internal ethics committee of Astellas Pharma, Inc., for review and approval as a minimal risk study (approval number: 000420–00).
