: Regional ethics committee approval was not required under Danish law. Individual-level data was stored only at Statistics Denmark. All analyses were performed on aggregated data that did not identify any individual. Data handling followed the guidelines of Directive 95/46/EC (General Data Protection Regulation) and the Danish Data Protection Act (Databeskyttelsesloven).
: This study did not involve any animal subjects. As it is a register-based study conducted in Italy, ethical approval is not required.
