: Data from the current study were collected in a clinical context as part of routine program evaluation and determined not to require ethics approval by a university institutional review board. This research was conducted in accordance with the International Ethical Guidelines for Health-related Research Involving Humans developed by the Council for International Organizations of Medical Sciences (CIOMS) in collaboration with the World Health Organization (WHO).
: Owing to the clinical nature and program evaluation context of the current study, a university institutional review board determined that this study did not meet the definition of human subjects research.
