: This study’s procedures were reviewed and approved by two university ethics committees from South Africa (ref 191001) and the United States of America (ref IRB2019-00567). Each participant received a reimbursement of R250 (US$16) for their time. Given the sensitive nature of the interviews, a distress protocol was in place, and a counsellor was available to provide psychosocial support to those who required it.
