: Matthew Macaluso, D.O., has conducted clinical trials research as principal investigator for the following pharmaceutical companies over the last 12 months: (1) Acadia, (2) Allergan, (3) AssureRx, (4) Eisai, (5) Lundbeck, (6) Janssen, (7) Naurex/Aptinyx, (8) Neurim, and (9) Suven. All clinical trial and study contracts were with and payments made to the Kansas University Medical Center Research Institute, a research institute affiliated with Kansas University School of Medicine- Wichita (KUSM-W).The other authors declare no conflict of interest.
: All procedures performed in studies involving human participants were in accordance with the ethical standards of the KUMC Human Research Protection Program (STUDY00143849) and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards.
: Informed consent was obtained electronically prior to starting the survey. This included the length of the survey, what information would be obtained, how the data would be used, IRB approval, and risks/benefits of participation. Participation was voluntary and participants were allowed to stop at any time if they wished not to continue. All data collected was stored in the secure REDCap database. No personally identifiable information was obtained.
