: At the time of data collection, ethical approval by the ethics review board was not required under Dutch law. Following the current guidelines of the national Research Ethics Committee of the Netherlands Association for Medical Education and in accordance with the Declaration of Helsinki, participation in the present study was voluntary, all participants provided written informed consent, and data were analysed anonymously.

: Medical residents were contacted by mail (2005) and email (2015). In this (e)mail, we attached an information letter about our study and an informed consent form. Participants in 2005 could anonymously complete and return the questionnaire, either by post or online on a specially created website. Participants in 2015 could use the Internet link in the email to anonymously complete and the questionnaire online.

: Personal data were stored coded. Our manuscript does not contain any individual person’s data in any form.
