: Informed oral consent was obtained from all participants prior to inclusion. The patients were informed about the purpose of the study. It was emphasized that their participation would not have any consequences for present or future treatment and that all data were pseudonymized.
: The Danish Data Protection Agency (Region Nord) (registration number 2008–58-0028) and Head of Research at the hospital approved the study. We further refer to the assessment from the Regional Ethics Committee.
