: This study was an educational project using anonymized cases. Ethics approval was sought and obtained from the local research ethics board. Our ethics board determined that informed consent was not necessary for this project because it was retrospective and all data collected was anonymized. Patients will not be able to be identified given data variables collected. It was impossible or impractical to obtain informed consent for this project given that many of cases were collected during the supervising physicians residency training (> 10 years ago), and it is possible that many of these patients would have expired due to their ages at time of data collection.
