: Before visiting the study sites for data collection, ethical clearance was secured from the research and ethics committee of the Federal Capital Territory Hospitals Management Board. Additionally, approval and access to relevant data were sought from the heads of the institutions. The focal person for pharmacovigilance within the institutions was approached to provide responses for the assessment of indicators.

: Before inclusion in the study, institutional approval was obtained from the management of the health facilities, and a consent form filled by the focal person for pharmacovigilance within the institutions before providing responses for the assessment of indicators.
