All research activities involving human participants were conducted in accordance with the requirements of the National Statement on Ethical Conduct in Human Research and the Australian Code for the Responsible Conduct of Research. Informed consent was obtained from all individual participants involved in the study.

: All participants involved in this study provided informed consent prior to their inclusion. For questionnaire participants, a consent form was provided at the starting page of the questionnaire. Only when participants agreed to take part in the research and completed the consent form was the rest of the questionnaire shown to them. For interview participants, a consent statement was provided prior to the start of the interview. Only when the interviewee signed the consent statement was the interview conducted.
