: Ethical approval was obtained from the National Health Research Ethics Committee of Nigeria with the code NHREC/01/01/2007. The project was guided by the ethical principles for medical research involving human subjects. The aims and objectives of the study were explained to participants after which written informed consent was obtained from each participant before proceeding with the survey or interviews. Participation was voluntary, and respondents were assured of the confidentiality of the information they provided. No personal identifying information, such as name and address, obtained from the respondents. Respondents were informed about the average length of time the interview might take. Data was saved on a password-protected computer and only designated investigators had access to it.
