: This research was approved by the Institutional Board of the University of Dar es Salaam, which is legally mandated to approve research conducted by students and staff of the University, under Ref. No. AB3/12/. Furthermore, approval was obtained from all local ethics committees. Formal written consent to participate in the study was sought from the involved health workers and health workers.
