: The study was performed in accordance with the ethical standards as laid down in the 1964 Declaration of Helsinki and its later amendments or comparable ethical standards. Ethical approval for the study was obtained from the Ghana Health Service Ethics Review Committee (ERC) with number GHS-ERC: 002/08/21. Permission was also obtained from the regional health administration of the two regions as well as the district health directorates of the selected districts. Informed consent was obtained from all participants. The contents of the checklist were explained to participants, and they were also provided with a printed participant’s information sheet in English prior to consent.
