: An opinion has been obtained from the Ethics Committee of Rhineland-Palatinate, Mainz, Germany, in accordance with the guidelines and recommendations for ensuring Good Epidemiological Practice (GEP). Due to the epidemiological nature of the study, no additional opinions from ethics committees were required.
