:
: There are no competing interests.
: Ethical approval was obtained from the regional ethics review board in Stockholm, dnr 2018/717-31/2.
: The feasibility of this study relied on refraining from the customary informed consent practice. Subjects were while treated identified through temporary identification numbers given to asylum seekers. The study carried out in retrospect and the subjects now either permanent residents or expelled, the numbers are no longer valid and hence tracking the subjects not possible. The practice not involving informed consent was approved by the Ethics review board.
