: This research study was conducted on secondary data obtained in a previous preference elicitation study. The primary study from which the data were obtained did not fall under the scope Medical Research Involving Human Subjects Act in the Netherlands and received a formal exemption from ethical review from the Erasmus Medical Centre’s Ethical Committee. The current study has a similar aim as the primary study and therefore no formal ethical review was pursued.
: This is a secondary data analysis on anonymized data collected in 2012, via a market research company. Respondents agreed to the terms and conditions of the market research company, were informed of the purpose of the study, and told during the interview that they would be able to stop the interview at any time if they would like. The respondents were not explicitly asked for informed consent. However, given the anonymous nature of the primary publication on the data, the similar aim of the current study, and the anonymized handling of the data in the current study, consent is not needed.
