: The Health Research Authority gave ethical approval to the University of Oxford for this study, and it was thereby in accordance with the ethical standards laid down in the 1964 Declaration of Helsinki and its later amendments. The participating NHS organisations confirmed their capacity and capability to host this research.
: AT and CC are affiliated with the Oxford Mindfulness Centre and employed to conduct research on MBCT and related psychological interventions. However, they receive no additional remuneration for books or training related to MBCT. PB is employed as the Mindfulness Lead for Tees, Esk and Wear Valleys NHS Foundation Trust. PB and TS receive payments for MBCT-related training and supervision. CS and RM co-lead the Sussex Mindfulness Centre and CS has received NIHR funding for research trials evaluating mindfulness-based interventions. WK is the director of the Oxford Mindfulness Centre. WK receives payments for training workshops and presentations related to MBCT and donates all such payments to the Oxford Mindfulness Foundation, a charitable trust that supports the work of the Oxford Mindfulness Centre. WK was until 2015 an unpaid Director of the Mindfulness Network Community Interest Company and gave evidence to the UK Mindfulness All Party Parliamentary Group.
