: This study was ethically approved by the Zhengzhou University’s Bioscience Ethical Review Committee, and the School Health and Education Programs Directorate of the Ghana Education Service with reference number (3/13/V.1/52). Additional permissions were granted by the Ashanti Regional Directorate of Education with reference number (14/90/33), Eastern Region with reference number (GES/ER/SS 332/VOL.3), and Brong Ahafo Region with reference number (GES/ASH/SDA/06/12).
: Official letters were dispersed to heads of selected institutions, and verbal assent sought from all participants who were 18–22 years before enrolling them in the study. Written parental assent was sought from parents/guardians of students who were 12–17 years at parent–teacher association (PTA) meetings of each of the selected schools before those students were enrolled in the study. For anonymity, names of schools or participants are not encompassed in the analysis.
