: The study was approved by the Institutional Ethics Committee of ICMR—National Centre for Disease Informatics and Research, Bengaluru which was the coordinating center as well as one of the survey’s implementing agencies (approval no: NCDIR/IEC/2017/4 dated 03 February 2017). Every implementing agency obtained its ethics approval from its own Institutional Ethics Committee before starting the survey.
