: This case report was written in compliance with our institutional privacy policy, the Health Insurance Portability and Accountability Act (HIPAA) policy, the standards set by the Declaration of Helsinki, and the Gaylord Hospital IRB. Institutional review board approval was not required by institutional policy as the report only describes a single patient; the need for approval was therefore waived by the Gaylord Hospital IRB.
