Recruitment for all sites for UMC-U was approved by their respective local Institutional Review Boards. Participants with mental capacity to consent provided written informed consent to study procedures. At the KCL centre, the study was also open to participants lacking capacity to consent if a consultee advised assent on their behalf (see Supplementary Methods).
