: The study protocol was approved by the Institutional Review board of the leading institution (General University Hospital in Prague, Czech Republic) in 10/2017 under the registration number 1587/17 S-IV. Due to the retrospective nature of the study, a need for informed consent was waived by the Institutional Review board. The study was performed in accordance with the Declaration of Helsinki.
