: The study had appropriate ethical approval for the use of tissue and anonymised clinical data from each of the four cohorts, including (1) Manchester Cancer Research Centre (MCRC) biobank (<i>n</i> = 34) (18/NW/0092); (2) a single centre Manchester retrospective cohort (<i>n</i> = 165) (06Q1403256); (3) the VORTEX-Biobank cohort (<i>n</i> = 203) (06/MRE03/3, NCT00423618); and (4) an intra-tumour heterogeneity cohort (<i>n</i> = 10 tumours, <i>n</i> = 45 biopsies, 3–8 biopsies/tumour) (06Q1403256). For the MCRC-Biobank and VORTEX-Biobank cohorts, the patients gave written informed consent. For the Manchester retrospective cohort, this was not required by the ethics committee as all clinical data were fully anonymised to the research team.
