Due to the retrospective nature of this study, we were not able to obtain written informed consent for the use of patients’ biopsy material and data for this study. Also, most of the patients had already died at the time of marker evaluation (see ref. <sup>69</sup>). In accordance with the Declaration of Helsinki, the use of the biopsy samples was done after consideration and approval by our institutional ethics committee of the Julius-Maximilians University of Würzburg, Bavaria, Germany, registration number: #103/14.
