: All methods were performed in accordance with the relevant guidelines and regulations. Peripheral blood or bone marrow samples were collected from patients after written informed consent was obtained. Studies were performed with the approval of the institutional Human Research Ethics Committees at WEHI (approval 05/04), the Royal Melbourne Hospital, the Monash Medical Centre, or the Alfred Hospital.
